To make a donation, please follow these steps:

 

1) Choose to download either the Excel form or the pdf document by clicking on the appropriate thumbnail link below;

2a) If you select the Excel file, you will be able to enter the information using your computer; then save and print it for mailing with your cheque;	2b) If you select the pdf document, you may print it out and fill it out by hand; then mail it with your cheque.
